Position title: Case Management Coordinator 

Reports to: Behavioral Health Director 

Classification: Full-time, Non-exempt 

Supervises others: No  

Salary: $24.57 - $29.34 

Location: 95825 - Sacramento, CA

Opening Doors began in 1993 as a small refugee resettlement agency, and today serves over 3,000 clients per year across our 5 program areas: Refugee Programs, Survivors of Trafficking Programs, Immigration Legal Services, Health Programs and Economic Prosperity Programs. 

Ideal candidate: The ideal candidate will be passionate about providing support and services to refugee clients. They will demonstrate strong interpersonal skills and provide direct case management to clients. 

They will have the ability to work effectively in multi-cultural work environments, showing respect and sensitivity for cultural differences, and will be passionate about Opening Doors’ mission. 

Responsibilities: The essential duties and responsibilities of this position include the following: 

  • Coordinate health, behavioral health, psychosocial, educational and housing services. 

  • Collaborate with Community Health Worker and develop a self-sufficiency and wellness plan for clients. 

  • Coordinate services and activities that are best fit for the clients. 

  • Supervise referrals and follow-ups to ensure timely and appropriate service delivery. 

  • Manage program emergency assistance and verify eligibility documents submitted by clients.
