Location: Trumbull County & Surrounding Areas (Warren, OH Region)
Schedule: Part-Time (20 hours/week) | Flexible, Field-Based
Compensation:

• $18/hr for Behavioral Health Marketing & Case Management (Comprehensive Behavioral Health Associates, Inc.)

• $14/hr for Home Health Services (Moonlight Home Health)

• Mileage reimbursement provided

This is a unique, part-time dual position supporting two sister organizations—Comprehensive Behavioral Health Associates, Inc. (CBHA) and Moonlight Home Health. The role combines community-based marketing and case management for behavioral health clients with in-home care for home health clients. The balance between these two responsibilities may shift based on client need and outreach priorities.

Marketing & Outreach (Guaranteed 4 hours/day):

• Promote CBHA’s behavioral health services through strategic outreach

• Build and maintain community partnerships and referral relationships

• Travel daily for in-person networking and events

• Create and share digital content (social media, email, website updates)

• Track outreach efforts and adjust strategies for effectiveness

Case Management (As caseload builds)

• Conduct intakes and assessments with new behavioral health clients

• Develop individualized care plans and provide ongoing support

• Coordinate referrals, transportation, and service access

• Maintain accurate, timely documentation in electronic systems

• Ensure client engagement and connection to needed resources

• Assist clients in their homes with personal care (bathing, grooming, dressing, toileting, mobility)

• Perform light housekeeping and basic meal prep

• Observe and report physical or emotional changes in clients

• Offer companionship and help maintain a safe, respectful home environment

• Travel regularly throughout the Warren area and surrounding counties

• Passionate about whole-person care — supporting both physical and mental well-being

• Self-directed and comfortable managing time independently while on the road

• Experienced in either or both of the following:

  • Marketing/outreach and case management (especially in behavioral health)

  • Home health or personal care services

• Strong communication, documentation, and interpersonal skills

• Able to build trust with clients and navigate multiple care systems

• High school diploma or equivalent required

• Valid driver’s license and reliable vehicle (daily travel is essential)

• Certification preferred (HHA, PCA, CNA) for home health portion

• Experience with EMR platforms and basic tech tools preferred

• Must pass background check, screenings, and complete relevant training
