Adult Group Home Caregiver
JOB RESPONSIBILITIES:
RESIDENT WELL-BEING
1. All residents are safe and feel loved and cared for
2. Be our advocate for each resident at their medical meetings and hospital stays when asked
3. Doctor visits are scheduled and transportation is coordinated
4. Staff is always aware of all necessary information regarding residents
5. All individual resident care plans are being followed
6. Ensure each resident’s activity, food intake, functional and psychosocial status are appropriate for their state of health and well-being
7. Follow all daily living protocols and guidelines
8. All resident emergencies are acted upon immediately, properly, are documented and appropriate management is informed
9. Document and ensure all staff and departments are aware of any needs required for residents’ well-being
10. Promote open communication between residents, health care professionals, families, and staff
11. You are always cheerful, friendly, and proactive with residents and their families
12. You have proper and adequate education & training, support and supplies to do your job well, including fire, disaster, emergency, and evacuation procedures. If not, you document and ask
HOME
1. Home has all the necessary supplies. If not, document and notify management
2. Monthly maintenance checks are performed, are documented and management is notified
3. Maintain clean and current medication administration records
4. Medications are locked up and safe at all times
5. Properly perform all duties, and if you don’t know ask for help. That includes working with other CGs and assistants as a team to complete daily duties including cooking, cleaning, resident care, laundry, etc.
6. Daily and weekly schedules are followed, including cleaning, resident activities, showering, menus, shopping, inventory, supplies, meds, work orders, vitals and glucose monitoring, MARS, logs, catheter care and logs
7. Work together as a team with House Assistant, Home Manager, and/or other Caregivers to keep the home clean and safe, and the residents happy and healthy.
8. Check to ensure all equipment is in safe and working order at all times. If not, document and notify management
ACTIVITIES
1. A variety of on- and off-site activities are scheduled, promoted and properly staffed
2. All residents and their families are aware of all the details about all on-site and off-site events & activities
3. All activities and events are administered without problems or negative feedback
4. All birthdays and their parties are known, planned for, staffed properly and as many family members as possible are present
5. All residents are getting safe and appropriate exercise
MAR
1. MARs are always up-to-date
2. New meds are listed, discontinued meds are purged and all meds are initialed
ADMINISTRATION
1. Always clock in and out properly and on time
2. Always are on-time, prepared and ready to work every shift
3. Always communicate to the next shift what happened on your shift and anything they need to know to do their job well
4. Always document shift notes in Basecamp
