POSITION DESCRIPTION

Essential Functions:

To assist clients, create and follow through with a career navigation plan consistent with helping clients accomplish goals and work towards self-sufficiency; mentoring clients; determine immediate and long-term needs, employability, and identify key barriers or assets in clients self-sufficiency; job coaching skills; communicate with service providers to evaluate client progress; offer counsel and guidance; maintain regular face-to-face contact with clients.

 

Responsibilities:

This job requires substantial knowledge of coaching and mentoring to build successful program participants who possess creativity, leadership ability and oral & written communication skills. The Career Navigator is responsible for identifying risks and consultatively develops preventative strategies, assist clients in learning life skills and development & implementation of a leadership framework.

 

1.      Maintain case documentation, i.e. client records, including progress notes and all other relevant information to assist clients and to comply with grant requirements; review and verify all documentation submitted to support request for assistance in accordance with program guidelines; explain to clients their rights, responsibilities, required actions, and other available services based on the clients’ ability to be successful.

2.      Create an Individual Enhancement Plan (IEP) and preform an interest assessment on each client.

3.      Suggesting appropriate development activity for program participants and assist in development of personal development plans

4.      Provide timely guidance and feedback to help strengthen specific knowledge/skill areas to accomplish the mentees objectives

5.      Conduct other case management activities, i.e. travel to assist clients with specific case management needs, prepare case summaries and reports, attend meetings and training sessions, and provide referrals to appropriate outside agencies.

6.      Monitor client progress though phone calls, site visits and case management appointments. Act as a liaison between program participants and project partners who are delivering supportive services.

7.      Maintain professional knowledge in applicable areas and keep abreast of changes in job-related rules, statues, and laws; read and interpret professional literature; attend workshops, meeting, and seminars if applicable.

8.      Keep the Program Manager informed of unusual and/or crisis situations as they arise.

 

 

Skills and Abilities:

 

1.      Must have excellent organizational skills and be able to complete projects efficiently and independently.

2.      Must have excellent communication skills including verbal, phone, word processing, email and internet.

3.      Must have good public relations skills and the ability to work with employees and the public.

4.      Must establish and maintain effective working relationships with, and among, all personnel.

5.      Must be able to read, write and communicate clearly in English.

6.      Must be able to work occasionally long or extended hour, including weekends.

7.      Must be able to function in a hectic work environment with occasional periods of high stress.

8.      Must maintain strict confidentiality of all information.

 

Requirements:

 1.      Minimum of BA or BS degree is required and one-year experience in criminal justice, social work, employment counseling, placement, human services, or public administration.

2.      Excellent oral and written communication skills.

3.      Must be proficient in operating a computer and experience with Microsoft Office (Word, Excel, PowerPoint, Outlook), and other social media applications.

4.      Must provide own transportation.

5.      Must be able to clear DMV check.

6.      Must possess valid operator’s license and maintain current vehicle registration, liability insurance and vehicle inspection.

7.      Must be ambulatory.

 

Physical/Mental Demands:

Must be able to tolerate frequent periods of high stress, heavy workload, and multiple priorities.   Must be able to see, hear, speak, and otherwise communicate effectively.
