·       Advocate on behalf of our clients, and identify appropriate interventions that promote a greater level of functioning for the client;
·       Record client information/evaluations and medical assessments and reports using various company technology systems;
·       Coordinate of client’s medical and healthcare provider services;
·       On-going research of governmental programs applicable to mental health other medical conditions (across Canada);
·       Ensure our client’s access, the services they need in order to achieve their goals and recovery, including scheduling appointments;
·       Explain medical procedures and/or treatment options;
·       Collect and distribute client information as required;
·       Other duties as assigned


·       This is a full-time position hours vary between Monday to Friday - 8:00 am to 6:00pm

Who You'll Work With:
Our current team is extensive & diverse, consisting of experts in their respective fields including but not limited to Registered Nurses/Registered Practical Nurses and Social Workers who have a passion for helping others. This role will report to the Team Lead MedEx/RTOERO

What you'll be doing: 
You will join an already fabulous team of 11 motivated, dedicated and unique professionals in working with clients and case managers in expediting specialist appointments!  The navigator manages everything involved in ensuring a successful consultation, appointment, treatment and/or surgery. There are many moving parts and to be successful you must bring an ability to empathize, multi-task, prioritize and support our clients and each other
·       Assess, manage, support and monitor client’s individual health needs

What you'll need to be successful:
·       Exceptional communication skills in English and French (written & verbal)
·       Additional training, experience & passion for mental health
·       Experience/education in disability case management
·       A dedicated, quiet home office and reliable Wi-Fi
·       Energy, integrity & thrive working in a fast-paced environment
·       Maturity and want the opportunity to use your years of experience in a different capacity
·       A wardrobe of comfy clothes and are not looking to go back to standard business attire – This is a permanent remote position;
·       RPN/RN in good standing (must be!)
·       Empathy and understanding
·       Highly proficient computer skills
·       Strong knowledge of the Canadian Healthcare system
·       Strong time management & organizational skills
·       Problem/symptom-based approaches and focus on solution-based interventions
·       Worked in roles with performance indicators and rise to the challenge to meet established metrics
·       Pride in being a positive team player, drama-free, tenacious, intuitive, caring, curious, FUN, is able to multi-task and prioritize competing priorities
