At All American Home Care, we are committed to providing compassionate, high-quality home health services to our patients. We are looking for a dedicated Admissions Coordinator to join our team and play a key role in ensuring a seamless intake process for new patients.

Job Summary:

The bilingual Admissions Coordinator is responsible for managing the intake and admissions process for new patients, ensuring accuracy, efficiency, and compliance with company policies and healthcare regulations. This role involves coordinating with patients, families, healthcare professionals, and internal teams to facilitate smooth transitions into home care services.

Key Responsibilities:

• Verify patient information. 

• Ensure all necessary documents are completed and filed.

• Work with the internal team to schedule patient assessments, appointments, and services according to the patient's care plan.

• Maintain up-to-date records for all admissions, ensuring that information is accurate and confidential.

• Communicate with patients and families, providing guidance and support throughout the admissions process.

• Work closely with other departments to ensure a cohesive process for new patient admissions.

• Ensure timely and accurate scheduling of patient care visits and follow-ups.

• Participate in regular team meetings to improve processes and ensure high standards of patient care

Qualifications:

• High school diploma or equivalent required; associate’s or bachelor’s degree preferred.

• Fluency in both English and Spanish  (written and verbal).

• Previous experience in healthcare or admissions is preferred.

• Strong organizational and multitasking skills with attention to detail.

• Excellent communication, problem solving and personal skills.

• Ability to maintain confidentiality and adhere to ethical standards.
