Position Overview:

As an LPN at Maple Crest, you will oversee the clinical care of elders in our assisted living community, ensuring their physical, emotional, and spiritual needs are met. You will collaborate closely with Elder Assistants, the ALC, and the Clinical Support Team to maintain high standards of care. Key responsibilities include:

  • Providing care in accordance with state and federal guidelines, Nursing Standards of Practice, and Maple Crest Nursing Policies and Procedures.
  • Conducting clinical assessments and monitoring elders’ health status during each shift.
  • Administering and documenting medications and treatments accurately.
  • Collaborating with Elder Assistants and supporting them in maintaining a positive care rhythm.
  • Participating in care planning, ensuring compliance with and updates to care plans based on elder needs.
  • Observing and reporting changes in elders’ conditions to the ALC, physician, and family.
  • Providing clinical teaching and guidance to Elder Assistants.
  • Acting as a role model in implementing MHCO/Maple Crest policies and OSHA Standard Precautions.
  • Maintaining emergency supplies and operating medical equipment as needed (e.g., oxygen equipment).
  • Performing CPR and the Heimlich maneuver, maintaining a current certification.
  • Assisting with personal care services and filling in for other nurses when necessary.
  • Participating in the on-call schedule and assisting with staffing as needed.

Qualifications:

  • Current LPN license in the State of Ohio.
  • Ability to recognize the autonomy and dignity of all elders.
  • Strong communication skills with a compassionate and caring attitude.
  • Ability to make prompt and accurate decisions regarding elder care and emergencies.
  • Flexibility, adaptability, and creativity in responding to changing situations.
  • Dependable and reliable with a commitment to quality care.

Preferred Qualifications:

  • Experience in a long-term care or assisted living setting.
  • Knowledge of gerontology and person-centered care practices.

Physical and Environmental Requirements:

  • Frequent walking and standing for up to 75% of an 8 to 12-hour shift.
  • Ability to lift, carry, and reposition elders weighing 75–350 lbs with or without assistance.
  • Ability to lift and carry light objects such as charts and treatment supplies.
  • Ability to bend, stoop, and reach overhead to obtain supplies.
  • Hand/eye coordination required for administering treatments and operating equipment.
  • Ability to read small print, distinguish sounds, and concentrate with interruptions.
  • Occasional exposure to blood, bodily fluids, and infectious diseases.
  • Exposure to hazardous chemicals, disinfectants, and personal protective equipment.

  • Report relevant information discreetly to the Director of Nursing (DON) or ALC as needed.
  • Comply with all Corporate Compliance Policies and the Code of Ethics.
