ACTT Therapist - Team Leader(LCSW(A)/LCMHC(A)

Pathways to Life, Inc. – Greenville, NC

Full-Time | Non-Exempt

Schedule: Monday–Friday: 8:00 a.m.–5:00 p.m.

Rotating 24/7 crisis coverage shared across the ACTT team.

Reports to: ACTT Team Leader

Role

You will provide therapy and clinical support to adults living with:

  • Severe mental illness

  • Co-occurring substance use disorders

Services follow the Assertive Community Treatment Team (ACTT) model. Treatment occurs in homes, shelters, hospitals, and community settings, not in a clinic-only environment.

What You Will Do

Clinical Services

  • Complete behavioral health and substance use assessments

  • Develop individualized treatment plans with the ACTT team

  • Provide individual counseling using Motivational Interviewing, Cognitive Behavioral Therapy, & Relapse prevention strategies

Community-Based Care

  • Deliver services in homes, shelters, hospitals, and community locations

  • Facilitate person-centered planning meetings

  • Support behavioral stabilization and anger management

Crisis Response

  • Participate in the ACTT rotating 24/7 crisis response schedule

  • De-escalate behavioral health crises in community settings

Team Collaboration

  • Coordinate care with psychiatry, nursing, employment specialists, and peer support

  • Maintain documentation that meets Medicaid and ACTT standards

Most services occur in the field with clients rather than at a desk.

Minimum Requirements

Licensure

You hold one active North Carolina license:

  • Licensed Clinical Mental Health Counselor (LCMHC)

  • Licensed Clinical Mental Health Counselor Associate (LCMHC-A)

  • Licensed Clinical Social Worker (LCSW)

  • Licensed Clinical Social Worker Associate (LCSW-A)

Experience

  • At least 1 year working with adults through employment or volunteer work

  • Experience supporting individuals with serious mental illness or behavioral health needs

Clinical Leadership

  • Ability to provide clinical guidance and support to a multidisciplinary team

Required Certification/License: LCMHC, LCMHC-A, LCSW, or LCSW-A

Work Conditions

  • Drive daily to client locations

  • Lift up to 25 lbs

  • Work in homes, shelters, hospitals, and office settings

  • Participate in evening, weekend, and holiday on-call rotation

You may encounter behavioral health crises in community environments. Crisis response training is provided.
