Job Summary

The Access Coordinator is responsible for the oversight of the centralized intake processes for all bed-based substance use programs. The Access Coordinator supervises the Intake Team who support intake, transition, and discharge planning across all bed-based programs.

 

Responsibilities

Program Management

§  Responsible for ensuring accuracy and completeness of client files to ensure adherence to established processes, accreditation, licensing and safety guidelines, and/or funding sources.

§  Ensures the Intake Team follow established criteria and prioritizes intakes based on criteria, leads, and facilitates actions related to daily client flow, plans for client transfer or discharge and appropriate client placement/referral

§  Liaises with the clinical care team to ensure specific care plan actions and processes are completed and reviews the intake and discharge planning of clients to ensure optimal progression to discharge, in collaboration with the other clinical team members

§  Provides oversight related to clinical and organizational data collection across all MHSU programs; prepares reports for MHSU leadership and funders, as required

§  Acts as a point person for clients, families, and community care teams who have concerns or questions regarding their care; follows ups as required

§  Performs other tasks and duties as required

 

Staff Supervision

§  Provides supervision, direction, coaching and support to the Intake Team in the completion of daily operating tasks and routines.

§  Orients, trains, supervises, and evaluates new Intake Team members on shift responsibilities and routines and reports to supervisory position.

§  Motivates staff team to maintain and exceed agency work standards. This position does not involve hiring, firing, or disciplining employees.

§  Ensures that all documentation for shift tasks, responsibilities and routines are completed.

§  Assigns and adjusts workloads and schedules, and coordinates staff coverage as needed to ensure service levels and staffing requirements are met

 

Human Resources

§  Assists the Manager by attending panel interviews, providing recommendations on the selection of new staff, providing feedback to the Manager on staff performance appraisals, and optimal use of budgetary resources

§  Assists the Manager in determining related training and orientation requirements

Community Engagement

§  Participates in funder meetings and community stakeholder committees on behalf of the program as required by the MHSU leadership.

 

Qualifications

§  Post-secondary diploma in social work, community services, etc., or an equivalent combination of education and experience

§  2 to 4 years of practical experience in an admission setting working with vulnerable populations, substance use services, mental health, or corrections.

§  Satisfactory completion of a criminal records check with vulnerable sector screening is required

§  Satisfactory completion of Tuberculosis Screening form is required

§  Satisfactory completion of Employee Immunization Record form is required

§  First aid certification with CPR is required

§  Valid driver’s license with driver’s abstract

§  High level of integrity and dependability with a strong sense of urgency and results-orientation

§  Demonstrated strong written, verbal and interpersonal communication skills

§  Ability to lead and work in a trauma-informed practice

§  Demonstrated commitment to the social sector with a passion for the organization’s mission is essential

Salary for this role is between $52,162 to $63,199 per year.
