ABH Case Manager

POSITION SUMMARY:  The Assisi Bridge House (ABH) is based on 12 recovery domains in cooperation with the program of Alcoholics Anonymous/ Narcotics Anonymous.  The Case Manager is responsible for working with the treatment team to connect clients to the services needed and be a link to resources in the community outside of ABH.

 ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING:

 (other duties may be assigned)

·       Be a liaison with Medicaid and SCLHSA providers to preauthorize and authorize stays for the clients at ABH based on their individual needs. This requires an understanding of diagnoses codes, ASAM criteria, client progress, family involvement and discharge planning.

·       Be able to manage the waiting list in regards to priority placement and exclusionary criteria. This also includes referring callers who do not meet the criteria of ABH to appropriate care.

·       Make referrals for clients to resources including but not limited to behavioral health, medical appointments, dental and eye care appointments, job placement center, LRS, parenting classes, smoking cessation and housing among others.

·       Make follow-up calls to former ABH clients to determine if they have one year clean and sober and obtain information regarding their status at that time (job, housing, etc.

·       Have a good understanding of charting.

·       Assist with research and calls for building repairs, maintenance and renovation.

·       Participate in group and individual sessions as needed/required to support clients in their recovery efforts.

OTHER OR SHARED RESPONSIBILITIES:    

·       Assist the administrator and clinical director with other program or administrative duties as requested.

 QUALIFICATIONS TO PERFORM THE JOB SUCCESSFULLY:

Education and/or Experience:

·       College degree preferred.

·       Peer support specialist certification preferred.

·       Must be familiar with all dynamics of alcoholism and drug abuse and the various programs for treating the disease.

·       Driver’s license required.

 Other Skills required:

·       All employees of ABH and ILP must maintain strict confidentiality al all times.
