#128-24 Transition Support Specialist- Wilton
SUMMARY:
The primary responsibility of the Transitional Support staff is to assist the program manager in the day to day administration and operations of the Transitional House Program. This position will ensure the goals and objectives of individuals and their parents/guardians; the Dept. of Development Service and the Dept. of Social Services are followed in compliance with their policies and procedures.
REQUIREMENTS:
- High school Diploma or GED. Knowledge of DDS systems.
- Valid driver’s license and reliable transportation.
- Ability to interact with people with intellectual disabilities.
- Knowledge of basic computer technology and software applications such as Word, Excel and Outlook.
RESPONSIBILITIES:
- Staff will communicate respectfully, professionally, effectively and with integrity.
- Provide DDS and CPM Supervisor with a plan of action to immediately ensure and provide transportation of the individual referred to the Wilton Transitional Home.
- Work with DDS to respond quickly to individuals referred.
- Assess the physical and emotional status of individual in crisis and respond appropriately.
- Communicate with individuals to ensure a welcoming, safe, and secure environment.
- Process necessary paperwork and record keeping, following all DDS protocols.
- Complete job tasks and worksite documentation in a timely manner.
- Provide input to the development of the individual program goals (IP).
- Engage individuals and suggest alternative strategies as appropriate.
- Commitment to safety in all areas, ability to respond to emergencies in a calm manner.
- Abide by all CPM/PFC policies and procedures, other duties as assigned.
SCHEDULES AVAILABLE:
Full time: Monday- Friday (3:00pm-11:00pm)
Part time: Saturday - Sunday (3:00pm -11:00pm)
Part time: Friday - Saturday (7:00am - 3:00pm)OR
