JOB SUMMARY

This position is responsible for case management activities of CCHN’s Transitional Housing Program to assist individuals and families who are experiencing homelessness.  Through the activities of intensive case management, community collaboration, and advocacy this position will work to ensure that all vulnerable people in this target population have their housing, medical, behavioral health, environmental health and other identified needs addressed.  This position will collaborate with community partners such as: Cayuga County Mental Health, Cayuga County DSS, East Hill Medical, City of Auburn, Cayuga County, Chapel House, and the Auburn Rescue Mission, etc.  to coordinate referrals, assist with applications, and ensure access to all needed resources.  This position reports to the Director of Street Outreach and Housing.

Salary-$39,000 plus benefits 

JOB DUTIES AND RESPONSIBILITIBS:

        Intake, case management, and discharge of all participants.

        Ensuring all required documentation is completed and maintained according to agency policies and procedures.

        Conduct daily wellness checks and random weekly checks of all participants.

        Referrals to match identified needs of all participants (DSS, behavioral or physical healthcare, care coordination, permanent housing, employment, SNAP benefits, healthcare, etc.)

        Assist with applications, phone calls, and landlord negotiations as needed.

        Review health and wellness issues and develop a Housing Wellness Plan that includes housing, resources, and services.

        Conduct random wellness checks one weekend day every other week to ensure safety and health.

        Work with area landlords to develop relationships that provide access to safe and affordable housing.

        Ensure families with school age children are connected to school districts per McKinney Vento provisions.

        Input and track all data in HMIS and any other data systems

        Complete referrals as needed and follow up with all referral sources to include referrals received and referrals made

        Ensure participant compliance with all rules and regulations of the program including the 90 day time limit of the program

        Responsible for preparing units for occupancy and ensuring all program supplies are available

        Attend relevant community meetings

        Work with Homeless Task Force to identify and address needs 

JOB REQUIREMENTS

        Bachelor's Degree in a health, human services or related field, Associates Degree will be considered with relevant experience

        Must have experience working with target population

        Must be able to work independently as well as in a team and be self-motivated, resourceful, organized and adaptable in carrying out job duties

        Must have good oral and written communication skills

        Must have competency with Office 365, and associated programs

        Must have demonstrated ability to work with a large cross-section of people in a warm, non-judgmental manner

        Knowledge of Motivational Interviewing and Harm Reduction skills

        Must be able to lift up to 50 lbs and be able to perform cleaning and setup of units

        Travel throughout Cayuga county is required; must possess a valid NYS Driver's License with driving record within agency policy and have access to a reliable vehicle.
