All American Home Care is seeking an experienced Patient Care Coordinator with proven home care operations experience. This role requires someone who thrives in fast-paced environments, understands EVV systems, and can manage high-volume caseloads with precision and professionalism.

Job Summary

The Patient Care Coordinator is responsible for managing a high-volume caseload, staffing caregiver shifts, handling urgent call-outs, and ensuring client satisfaction and retention. This position requires strong decision-making skills, operational control, and confidence under pressure.

Key Responsibilities

• Manage a caseload of 80–120 home care patients

• Staff caregiver shifts and ensure coverage for scheduled services

• Handle same-day call-outs and secure appropriate replacements

• Monitor authorization hours and address overutilization

• Work daily inside HHAeXchange / EVV systems

• Communicate with Service Coordinators and referral sources

• Resolve caregiver and client grievances professionally

• Review care plans with clients and caregivers

• Maintain accurate documentation and compliance standards

• Contribute to retention and overall branch performance

Required Qualifications

• Minimum 2 years of experience in a home care coordination or service coordination role

• Direct experience using HHAeXchange or other EVV systems

• Experience managing a caseload of at least 60+ patients

• Strong scheduling and staffing experience

• Ability to multitask under pressure

• Strong computer skills and documentation accuracy

• Bilingual English/Spanish preferred
