All American Home Care is seeking an experienced Patient Care Coordinator with proven home care operations experience. This role requires someone who thrives in fast-paced environments, understands EVV systems, and can manage high-volume caseloads with precision and professionalism.
Job Summary
The Patient Care Coordinator is responsible for managing a high-volume caseload, staffing caregiver shifts, handling urgent call-outs, and ensuring client satisfaction and retention. This position requires strong decision-making skills, operational control, and confidence under pressure.
Key Responsibilities
Manage a caseload of 80–120 home care patients
Staff caregiver shifts and ensure coverage for scheduled services
Handle same-day call-outs and secure appropriate replacements
Monitor authorization hours and address overutilization
Work daily inside HHAeXchange / EVV systems
Communicate with Service Coordinators and referral sources
Resolve caregiver and client grievances professionally
Review care plans with clients and caregivers
Maintain accurate documentation and compliance standards
Contribute to retention and overall branch performance
Required Qualifications
Minimum 2 years of experience in a home care coordination or service coordination role
Direct experience using HHAeXchange or other EVV systems
Experience managing a caseload of at least 60+ patients
Strong scheduling and staffing experience
Ability to multitask under pressure
Strong computer skills and documentation accuracy
Bilingual English/Spanish preferred
