Patient Care Coordinator
All American Home Care is seeking an experienced Patient Care Coordinator with proven home care operations experience. This role requires someone who thrives in fast-paced environments, understands EVV systems, and can manage high-volume caseloads with precision and professionalism.
Job Summary
The Patient Care Coordinator is responsible for managing a high-volume caseload, staffing caregiver shifts, handling urgent call-outs, and ensuring client satisfaction and retention. This position requires strong decision-making skills, operational control, and confidence under pressure.
Key Responsibilities
Manage and coordinate services for a caseload of home care patients while maintaining scheduling accuracy and service continuity.
Staff caregiver shifts and maintains full coverage for all authorized client services.
Respond to caregiver call-outs and schedule replacements while ensuring compliance with authorization hours.
Work daily inside HHAeXchange or another EVV system to monitor visits, scheduling accuracy, and service utilization.
Communicate with Service Coordinators, referral sources, caregivers, and clients regarding scheduling and service needs.
Monitor authorization hours and address utilization discrepancies before services exceed approved limits.
Document scheduling activities, service changes, and operational notes accurately in agency systems.
Support service continuity, caregiver retention, and overall branch performance through strong operational coordination.
Required Qualifications
Minimum 2 years of experience in a home care coordination or service coordination role
Direct experience using HHAeXchange or other EVV systems
Experience managing a caseload of at least 60+ patients
Strong scheduling and staffing experience
Ability to multitask under pressure
Strong computer skills and documentation accuracy
Bilingual English/Spanish preferred
