LPN / Quality Coordinator
Position Summary
Works under the direction of physicians or nurse practitioners. Assists in the planning, implementation, and evaluation of patient care. Performs nursing procedures and patient care tasks. Directly involved with Quality care and ACO implementation. Cooperates with the members of the clinical team and other departments of our medical practice. To be a positive professional image and good example of clinic practice.
Essential Duties and Responsibilities
The essential functions include, but are not limited to the following:
- Use of proper medical terminology when communication and documenting.
- Triages patients, checks vital signs, weight and accurately documenting in patient’s electronically medical record.
- Perform EKG’s, blood sugar check, and administer injections using safety devices.
- Properly reconciles and refills medications, documenting lot numbers and completing refill requests electronically.
- Prompt, concise, accurate and complete, communicating, and documenting questions, answers, and test results electronically and verbally.
- Accurately ordering and scheduling test and procedures electronically and on paper.
- Requests authorizations for procedures and surgeries indicating all the necessary diagnoses, dates of procedure in a timely manner and in advance allowing authorization representative to obtain proper approval.
- Professionally and accurately explain test procedures as well as internal protocols and procedures relating to patient care.
- Wound care, assist with properly wrapping patient wounds. Assist with educating patients about proper care to prevent infection.
- Effectively communicates frequently with the physician or nurse practitioner regarding patient’s physical and mental conditions.
- Implements, maintains, communicates Quality and HEDIS measures.
- Train and assist with on-boarding new clinical employees.
- Works as floor nurse when during a clinical staff shortage.
- Responds and assist with patient emergencies or incidents.
- Handles patient complaints relating to patient care
- Actively participate in and adhere to properly record quality care measure requirements.
- Accurately schedule appointments and coordinate surgeries and procedures.
Additional responsibilities:
- Participates in general, clinical safety and emergency care.
- Floats as requested by physician or managemen
- Assist in training new personnel as needed
- Performs other related duties as assigned.
- OSHA training
- Cleaning Spills of body fluid /infections
- Use of eye wash station
- Proper use of PPE
- Sharps lock down for disposal.
Minimum Qualifications (Knowledge, Skills, and Abilities)
High School graduate preferred.
Active LPN license.
Computer skills and experience working in an EMR/E.H.R, required.
CPR certified
Previous clinic or hospital experience with acceptable references preferred.

· Fast pace and ability to work under stressful situations.

· An individual in this position will be exposed to inside environmental conditions, blood borne pathogens, and respiratory hazards.
