A growing private duty home health company serving the Winter Haven and Lakeland area is looking for a Care Coordinator to join their team. This role sits at the center of operations, client experience, and care delivery—ideal for someone with healthcare experience who enjoys coordinating services, building relationships, and keeping things moving. This is not a hard-sales role. Experience in healthcare coordination, case management, or client-facing service roles is what matters most.

What You’ll Do

• Coordinate care services for private duty home health clients
• Serve as a primary point of contact for clients, families, caregivers, and referral sources
• Manage intake, scheduling, and ongoing care coordination
• Communicate with referral partners such as hospitals, discharge planners, and case managers
• Ensure care plans align with client needs and company standards
• Support client retention through strong service, follow-up, and problem resolution
• Maintain accurate documentation and records
• Assist with light outreach and relationship-building (no aggressive sales expectations)

What We’re Looking For

• Healthcare background required (home health, hospice, case management, SNF, hospital, or similar)
• Experience in care coordination, case management, intake, or patient services
• Strong customer service skills with a relationship-driven mindset
• Comfortable communicating with families, clinicians, and referral sources
• Organized, detail-oriented, and able to manage multiple cases at once
• Sales, intake, or client-facing experience is a plus, but direct home health sales is not required
