Summary

The  Case Manager - Independent Living with Supports will provide leadership, development and implementation of care plans for youth placed in the Independent Living Program. Contribute to the oversight of daily operations, financial management of case load and development of the program. To provide counsel for youth on a variety of issues to assist them in reaching set goals related to skill development (life skill, employment and educational). 

PRIMARY RESPONSIBILITIES:  of the Case Manger - ILS include, but are not limited to:

Administrative / program related:

  • Provides case management for youth in the Independent Living with Supports Program 
  • Manages monthly finances and expenditures for you on the case load 
  • With assistance from program Occupational Therapist develops Risk Assessments, Safety Plans and Care Plans
  • Meets regularly with the Program Manager/Occupational Therapist for the purpose of collaboration, planning and clinical oversight
  • Ensures a systems approach to planning
  • Ensures collaborative (youth directed) discharge and transition planning
  • Provides supervision and mentorship to the Tenancy Support Mentor (TSM)
  • Schedules support staff(s) as per youths' needs
  • Completes timely and detailed record keeping; ensures TSM does the same
  • Provides crisis management 
  • Coordinates and supports all medical, therapy and external appointments 
  • Coordinates communication with the placing agency, external collaterals and with other Link services and programs as needed
  • Ensures participants home/apartment is up to residential tenancy standards and are suitable for the individual youth, including furnishings and other basic living necessities
  • Develops relationships with landlords, including follow up after complaints, evictions, etc. 
  • Assists youth to search for, view and apply for apartments 
  • Prepares leasing application packages for Management companies, connecting with The Link CEO or representative and The Link accounting department to gather necessary paperwork and damage deposit
  • Completes move in/move out condition reports with Management company or caretaker of building
  • Attends staff meetings and contributes to the overall functioning of the agency by assuming appropriate additional responsibilities as needed
  • Displays an attitude that promotes positive teamwork and exhibits a high degree of positive role-modeling
  • Provides service through a lens that is holistic and focuses on the strengths and experiences of youth 
  • Other duties as assigned and required 

QUALIFICATIONS:

Education: 

  • Post-secondary degree in Education or Human Services field preferred 

Experience: 

  • Minimum of two years related experience working with high-risk youth
  • Proven ability to network and create positive community relationships and partnerships 
  • Demonstrates effective leadership attributes 
  • Understanding and appreciation of impact of our history related to colonization and outcomes from the Truth and Reconciliation Commission including Calls to Action 
  • Excellent organizational, interpersonal and communication skills 
  • Demonstrated proficiency working with Microsoft Office at an Intermediate level 
  • Strong interpersonal skills 
  • Knowledge of community resources
  • Knowledge of Strengths based and Solution Oriented approaches 
  • Crisis intervention, harm reduction techniques and conflict resolution skills 

Required 

  • Ability to be scheduled base on program and participant needs (days, evenings and weekends)
  • Valid drivers license and drivers abstract
  • Access to a vehicle for work purposes - mileage is paid for - travel will be required in surrounding areas in and around Winnipeg
  • Vehicle insurance should be all-purpose and required third party liability limit
  • Criminal reference and abuse registry check (subject to approval)
  • Willingness to obtain necessary training as per agency training policy including: CPR/First Aid Level C, De-Escalation Training, Suicide Intervention, Mental Health First Aide, Safe Talk and Child Development
  • Satisfactory Criminal Record Check with Vulnerable Sector Search, a clear Child Abuse Registry check with acceptable Prior Contact checks and a clear Adult Abuse Registry check, all done within the last 3 months


SALARY RANGE:  Starts at 46,800 per annum, depending on qualifications. The Link offers a benefit package, paid sick time, vacation and a pension.

HOURS OF WORK:  40 hours per week - Monday-Friday (a combination of days, evenings, weekends)
