Case Manager - Housing First

Kelowna, BC Full-time $24.36-$28.01/hour

CMHA Kelowna is hiring a full time Case Manager. This position offers 37.5 hours a week with a typical work week of Monday – Friday, 8:00am – 4:00pm.

 

WHO IS A HOUSING FIRST CASE MANAGER?

The Case Manager (CM) in the Housing First Program assists with addressing the needs of participants by providing them with immediate access to housing that is safe, affordable and appropriate, and the necessary and appropriate supports that focus on health, well-being, life skills, engagement in education and employment, and social inclusion.

WHAT DOES A HOUSING FIRST CASE MANAGER DO?

  • Provides intensive Case Management services and support to a small caseload of clients (7-10 maximum) that are experiencing, or at significant risk of, being unhoused
  • Collaborates with participants in creating individualized support plans of care unique to each person and reviewed and/or modified regularly
  • Provides case management, outreach, advocacy and access to information and other services
  • Provides services that address the unique needs of clients, such as developing life skills, navigating systems and services, integrating mental health and addictions supports
  • Provides services in the client’s home and in the community, transporting clients to appointments and other resources
  • Moves clients in and out of their homes including packing, carrying, and moving client belongings
  • Meets regularly with the Manager and team to review caseloads and troubleshoot client concerns
  • Facilitates connections and referrals to other community services, including medical facilities, mental health services, social assistance programs, and substance use services
  • Provides an ethical, efficient and therapeutic approach to dealing with crisis intervention in a timely manner
  • Collaborates with emergency response teams, law enforcement, and medical personnel when necessary
  • Maintains written and computerized records, including case notes, data/statistical information, and other relevant documentation using databases
  • Works within the program budget, monitors expenditures, and assists with financial reports
  • Provides information for reporting on grants and funding to the Manager as needed
  • Performs other related duties as assigned

YOUR STRENGTHS & EXPERIENCE:

  • Degree is Social Services or related field
  • Experience working with people with serious mental illness and/or addictions and those that have experienced homelessness required
  • Valid Class 5 driver’s license
  • Criminal Record Check clearance in the Vulnerable Sector
  • Reliable vehicle and ability to travel between facilities and other locations on a daily basis
  • Minimum 2 years of Case Management experience preferred
  • Certificates in First Aid, CPR, and Naloxone considered an asset
  • Ability to engage respectfully and develop rapport with individuals, with multiple barriers, through a harm reduction lens
  • Communication skills to effectively support clients
  • Ability to work in cooperation with team members and independently
  • Understanding of and commitment to working with people experiencing mental health concerns and substance use
  • Knowledge of current best practices and approaches in preventing and ending homelessness
  • Knowledge of health and social service systems
  • Ability to complete tasks in a timely and efficient manner, utilizing good organizational skills
  • Ability to manage multiple cases efficiently and effectively
  • Ability to maintain consistent professional boundaries, exercising mature judgment and understanding of safety concerns
  • Understanding of Harm Reduction theory and demonstrated ability to implement harm reduction strategies
  • Understanding of Intentional Outreach, Housing First, Rapid Re-Housing, and Shelter Diversion principles
  • Understanding of working with a Trauma Informed lens
  • Familiar with or ability to learn Homeless Individuals and Families Information System (HIFIS)
  • Proficient understanding of Microsoft Outlook, Word and Excel
  • Strong attention to detail
  • Critical thinking, problem-solving, and decision-making skills
  • Conflict resolution and crisis intervention skills
  • Ability to be regularly exposed to individuals who are struggling with and/or recovering from trauma, drug/alcohol use, homelessness, mental health challenges and/or mental illness
  • Ability to be regularly exposed to and in direct contact with dust, dirt, germs and human bio-products
