Description:

The Home Health Clinical Liaison is responsible for assisting in transitioning care for patients from facility to home through coordination of care between hospital/skilled nursing facility, home health agency, and physician.

 

JOB DUTIES/ KNOWLEDGE:

1.Reviews physicians’ orders and patients’ clinical needs, and gathering clinical information to transition referred patient to home.

2.Transfers patients from hospital/skilled nursing facility to home care services by establishing and maintaining relationships with referring hospitals; consulting with nursing, social services, hospital business office, physical and occupational therapy, and other support services.

3.Resolves patient care issues by working one-to-one with Clinical Supervisors/Director of Nursing to standardize patient home care assessments; collecting relevant information; conferring with co-caregivers; assessing patient home-care needs.

4.Educates hospital/skilled nursing team by attending nursing team meetings; providing orientation/in-service programs concerning home care intake coordination and hospital/skilled nursing facility relations; providing input relating to clinical concerns for individual patients.

5.Provides information by responding to queries of hospitals/skilled nursing facilities, attending physicians and their practice staffs; sorting and distributing messages and documents; answering questions and requests; preparing information for patient transfer; preparing statistical reports related to referral and intake activities from assigned hospitals/skilled nursing facility; maintaining databases.

6.Keeps hospital/skilled nursing facility and attending physician informed of patient status by monitoring and reporting home care services rendered and/or modified; monitoring home care progress; documenting emergency department episodic illness care visits; following-up on out-patient reports, and other patient information; anticipating additional home care services needed, i.e., infusion therapy, wound therapy, physical therapy, psychiatric care and/or other specialty programs.

7.Establishes hospital home care policies and procedures by consulting to hospital staff; providing state-of-the-art thinking in home care policies and procedures; facilitating focus groups; surveying public opinion; positioning the agency to serve the hospital/skilled nursing constituency.

8.Improves quality results by studying, evaluating, and re-designing home care procedures and patient progress in conjunction with the Director of Nursing/Administrator/Director of Business Development; recommending changes in home care and out-patient services.

9.Contributes to home care program effectiveness by identifying short-term and long-range issues that must be addressed; providing information and commentary pertinent to deliberations; recommending options and courses of action; implementing directives.

10.Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.

11.Enhances home care agency (service) reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.

12.Participates in the agency’s quality improvement program.

QUALIFICATIONS:

1.Degree in nursing (A.D.N. or B.S.N or L.V.N or L.P.N) from a program accredited by the National League for Nursing.

 

2.Licensed to practice professional nursing in the state.

 

3.Minimum of one year of nursing experience required.

 

4.Knowledge and Abilities:

 

a.Demonstrated knowledge and skills necessary to provide care to and communicate with primary population served by the agency, and to lesser degree the secondary populations served.

b.Demonstrated knowledge of the principles of growth and development over the life span.

c.Able to assess data reflecting the client’s status and interpret the appropriate information needed to identify each client’s requirements relative to their age-specific needs.

 

DEGRESS OF TRAVEL:

Daily travel to hospitals/skilled nursing facilities for patient/family meetings, nursing conferences.

Must have reliable transportation and agency required liability insurance. 

Must have a valid Texas Driver’s License*

DEGRESS OF DISRUPTION TO ROUTINE, OVERTIME:

Must be able to adapt to client status and needs, as directed. Schedule changes daily due to staffing, client condition, new clients, etc.
