Physical Therapist – Skilled Nursing Facility

Location: Orange, NJ

A skilled nursing facility in Orange, NJ is seeking a licensed Physical Therapist to provide high-quality rehabilitative care to short-term rehab and long-term care residents. This role is ideal for a Physical Therapist with skilled nursing or post-acute experience who is comfortable working as part of an interdisciplinary care team.

Key Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans
• Provide therapy services to short-term rehab and long-term care residents
• Collaborate with nursing, therapy, and medical staff to support resident goals
• Monitor patient progress and adjust treatment plans as needed
• Document evaluations, progress notes, and discharge summaries in compliance with regulations
• Participate in care plan meetings and interdisciplinary rounds
• Support safe discharge planning and functional goal achievement
• Ensure therapy services meet Medicare, Medicaid, and facility documentation standards

Required Qualifications

• Licensed Physical Therapist (PT) in the state of New Jersey
• 1–2+ years of experience in a skilled nursing facility, long-term care, or post-acute rehab setting preferred
• Strong knowledge of skilled documentation and Medicare Part A / Part B guidelines
• Ability to manage a productive caseload in a fast-paced environment
• Excellent communication and teamwork skills
