JOB SUMMARY
Siloam Mission’s Medical Transitional Care Unit (MTCU) is a compassionate 32-bed facility supporting individuals experiencing homelessness who have medical needs that do not require hospitalization. The MTCU provides time-limited transitional housing integrated with essential medical and social supports to improve health and housing outcomes.
Guided by person-centered, strengths-based, trauma-informed practice, and rooted in harm reduction and Housing First principles, the MTCU operates through a holistic and multidisciplinary approach.
The MTCU Manager is responsible for the overall leadership and daily operations of the unit, overseeing clinical and non-clinical staff, and ensuring that all residents receive comprehensive, integrated care from admission to discharge. The Manager works collaboratively with internal departments, hospital partners, community agencies, and the Siloam Health Centre to ensure residents are supported medically, socially, and in securing permanent housing.
KEY DUTIES & RESPONSIBILITIES
Program Leadership & Operations
- Lead and manage the MTCU, ensuring safe, effective, and high-quality service delivery
- Oversee all aspects of unit operations, including staffing, scheduling, workflows, and space utilization
- Embed low-barrier, harm reduction, Housing First, trauma-informed, and person-centered practices
- Monitor program performance and lead continuous quality improvement initiatives
- Ensure the program aligns with Siloam Mission’s mission, values, policies, and strategic priorities
- Develop and maintain professional collaborative relationships with department personnel, other departments within Siloam Mission, service providers, participants, and community resource agencies
- Exemplify Siloam’s core values in all day to day activities
Clinical & Interdisciplinary Oversight
- Provide leadership to clinical and non-clinical staff, including but not limited to nurses, therapists, case workers, intake & transition workers, and health care attendants
- Support interdisciplinary collaboration, case conferencing, and care planning across the full resident journey
- Ensure clarity of roles, scope of practice, delegation, and consistent implementation of processes
- Oversee clinical initiatives, including coordination of Nurse Practitioner services, pharmacy supports, and Opioid Agonist Treatment (OAT) programming
- Ensure residents receive integrated support throughout their stay, from referral to discharge
- Work collaboratively with the Intake & Transition Worker and other team members to maintain smooth transitions and continuity of care
- Oversee connections to internal Siloam supports, community resources, and housing services
Staff Leadership & Development
- Supervise, and evaluate staff, providing coaching, performance management, and professional development
- Support recruitment by monitoring staffing levels and initiating job posting requests, screening applicants, and participating in interviews and candidate selection
- Provide a robust onboarding program and process
- Foster a positive, inclusive, and psychologically safe work environment
- Support staff capacity to work effectively in complex, high-stress care environments
- Mediate conflict between staff or staff/participant as needed or upon request
- Address staff concerns in a timely and appropriate manner
- Assist leadership team in the implementation of new policies and practices
Harm Reduction, Safety & Risk Management
- Ensure consistent application of harm reduction and trauma-informed approaches throughout the unit
- Oversee safety protocols, including overdose response, crisis intervention, infection control, and incident management
- Review critical incidents and implement corrective actions as needed
Policy, Compliance & Resource Management
- Ensure compliance with legislation, regulatory requirements, funder expectations, and organizational policies
- Oversee documentation, privacy practices, and quality standards
- Manage program resources, including budgets, staffing, and supplies
Stakeholder Engagement & Advocacy
- Represent the MTCU with external partners, service providers, funders, and community organizations
- Foster collaboration with volunteers, hospital partners, and external agencies
- Develop and maintain professional collaborative relationships with interdepartmental personnel, other departments within Siloam Mission, and participants
- Advocate for resident needs and program resources to support effective service delivery
EDUCATION & EXPERIENCE
- Completion of a diploma or degree in health or human services (e.g., social work). Combination of education and experience may be considered
- Certifications in NVCI, ASIST, CPR/First Aid, Mental Health First Aid preferred
- Minimum 4 years in a leadership or management role
- Minimum 3 years in the social services sector
- Experience implementing harm reduction and trauma-informed approaches
- Knowledge of medical, mental health, and addiction systems
- Experience with budgeting, staff development, and program operations preferred
- Experience in Accuro software system preferred

RELATIONSHIP
- Direct supervision of MTCU staff
- Liaison between Siloam Mission, hospital partners, and community service providers
- Collaborates with Siloam leadership and Health Centre leadership
- Provides reports to senior management, funders, and the Board as required
- Daily interaction with residents
WORKING CONDITION
- Hours: Monday to Friday 8:00 a.m. – 4:30 p.m., some evenings and weekends may be required
- Part of Housing on-call rotation
- Office-based work with regular presence in the MTCU
- Exposure to substance use, mental health crises, medical needs, and emotionally demanding situations
- Potential for unpredictable or risky behaviors from residents or community members
WORKPLACE SAFETY AND HEALTH
The incumbent contributes to making the organization safe for clients and staff and recognizes the importance of reporting unsafe situations and participating in follow-up reviews as a learning opportunity.
- Provides a safe environment by ensuring adherence to Workplace Safety and Health Regulations and Policies, Infection Control Guidelines, WHMIS and Safe Work Procedures
- Immediately investigates and recommends corrective action on any unsafe acts, work conditions, incidents, near misses, injuries or illnesses
- Demonstrates understanding of role and responsibilities in fire prevention and disaster preparedness and participates in safety and health training programs including the facility’s Fire, Disaster and Evacuation Plan
- Supports an environment which avoids, prevents and corrects all activities or actions which may result in an adverse outcome in the delivery of services. Community Member safety is a standing item for all individual and departmental meetings
