Compassionate Hospice Nurse – Weekends - Winston Salem

 

We have made it our mission to provide compassionate care for individuals living with a life-limiting illness and their families, through quality medical, emotional, spiritual, and social support wherever care is needed. Our staff has grown from a dozen individuals to over 350 specially trained professionals who work as a team to help patients live fully and comfortably, with hope — even during life’s most challenging time.

 

Various Schedules Available

 

  • Full-time RN Weekends    Fri eve 4p-12midnight andSat & Sun  8a-8p  (32 hrs) OR Fri eve and Sat & Sun 8p-8a (32 hrs)
  • Part-time RN Weekends   Sat & Sun  8a-8p OR 8p-8a (24 hrs)

(every weekend or every other weekend)

                                                                                   

 Responsibilities

  • Triage patient related calls and make home visits based on assessment or patient/family’s request

  • Provide competent and compassionate nursing services including assessment, treatment, preventive measures and education and according to the plan of care (POC)

  • Strong nursing skills in wound care, lab draws, and PCA Pump Accuracy.

  • Document current patient status in a manner that care needs are identified, and hospice eligibility is evident

  • Identify/implement interventions and maintain the POC

  • Work with IDT/Team Leader in identifying patients for complex case management review

  • Provide emotional support about end of life/disease progression and symptom management

  • Recognize the influence of culture, ethics and beliefs in clinical decision making.

  • Inform the patient’s physician and IDT of any pertinent change in their condition

  • Compassionate and exceptional patient care in accordance with POC

  • Exceptional communication with patient and family to develop and follow POC

 

Requirements

  • Maintain a positive culture that is built upon commitment to patients, families and co-workers
  • Demonstrate uncompromising respect for patients, families and co-workers
  • Excellent nursing assessment and problem solving skills
  • Excellent interpersonal communication skills, both oral and written
  • Exceptional customer service
  • Strong clinical assessment, critical thinking, problem solving and organizational skills
  • Ability to utilize independent judgment and to follow through
  • Current NC license as a Registered Nurse and current CPR certification 

  • A minimum of two (2) years’ clinical experience preferred
  • Comfort with documentation within electronic medical record, experience with Allscripts/NetSmart a plus
