Cleveland Health Care Center is seeking a skilled and detail-oriented Wound Care / Treatment Nurse to join our clinical team on a weekends-only schedule. This role is ideal for an RN or LVN with long-term care experience who is passionate about skin integrity, wound prevention, and regulatory-compliant documentation.

This position plays a critical role in supporting survey readiness, quality outcomes, and resident comfort, while also providing general nursing support as needed.

The Wound Care / Treatment Nurse is responsible for coordinating and providing comprehensive skin and wound care services in accordance with state and federal regulations, facility policies, and standards of practice. This role ensures timely weekly skin assessments, accurate wound documentation, and clear communication with the interdisciplinary team.

The Wound Care Nurse also supports staff education related to skin integrity and participates in the facility’s wound prevention and management program, while maintaining the ability to perform general nursing duties as needed.

• Complete and document weekly skin assessments for all residents, identifying changes in condition, pressure areas, or skin breakdown

• Perform and document weekly wound observations for residents with pressure injuries or other skin impairments

• Accurately stage, measure, photograph, and describe wounds per policy and best practice

• Develop, implement, and update wound treatment plans in collaboration with the physician, DON, and interdisciplinary team

• Administer ordered wound treatments and dressings; monitor for signs of infection or delayed healing

• Ensure all skin and wound documentation is complete, accurate, and survey-ready

• Support pressure injury prevention measures, including repositioning programs and support surfaces

• Coordinate with dietary, therapy, and nursing staff to promote wound healing

• Track and trend wound and skin data; participate in QAPI activities and wound rounds as assigned

• Educate nursing staff on wound care techniques, infection prevention, and skin integrity best practices

• Collaborate with physicians, the medical director, and wound care consultants as needed

• Participate in interdisciplinary meetings and care plan reviews

• Provide direct resident care and medication administration as needed

• Respond to emergencies promptly and effectively

• Support facility policies, infection control practices, and resident rights

• Participate in surveys, audits, and inspections as directed

• Current, active RN or LVN license in good standing

• Minimum of 1 year nursing experience in skilled nursing, long-term care, or similar setting preferred

• Wound Care Certification (WCC, WOCN, or equivalent) preferred, but not required

• Knowledge of wound care principles, infection control, and documentation standards

• Strong communication, organization, and critical-thinking skills

• Ability to work independently and collaboratively within an interdisciplinary team

• Commitment to resident-centered care and quality improvement

• Ability to stand, walk, bend, and lift up to 50 lbs

• Visual and auditory acuity sufficient to assess and monitor resident conditions
