At All American Home Care, the Patient Care Coordinator ensures clients receive consistent, reliable home care services while maintaining compliance and operational accuracy. This role serves as the central point of coordination between clients and caregivers. You help ensure schedules are covered, documentation is accurate, supporting daily scheduling, coordination, and communication. If you are organized, detail-oriented, and comfortable working in a fast-paced healthcare environment, this role offers meaningful impact and operational responsibility.


What You’ll Do

  • Coordinate and maintain caregiver schedules to ensure full client coverage

  • Communicate daily with clients, families, caregivers, and other departments

  • Manage call-outs and last-minute coverage changes with urgency

  • Monitor open cases and assist with staffing adjustments

  • Utilize HHAeXchange (HHAx) to manage schedules and visit records

  • Monitor EVV (Electronic Visit Verification) compliance

  • Correct missed punches and address visit verification issues in a timely manner

  • Maintain accurate documentation in internal systems

  • Ensure professionalism, confidentiality, and compliance at all times

What You Bring

  • Experience in home care, healthcare coordination, scheduling, or case management (preferred)

  • Hands-on experience with HHAeXchange (HHAx)

  • Strong understanding of EVV requirements and visit verification processes

  • Strong communication and problem-solving skills

  • Ability to stay organized in a fast-paced environment

  • Comfort using scheduling systems, spreadsheets and internal platforms

  • Bilingual English/Spanish (a plus)

  • Reliable, detail-oriented, and team-oriented mindset
