    We are looking for an experienced MDS Coordinator (RN) to be part of a larger, cohesive team of 3 MDS Coordinators, supporting the Masonic community at our beautiful campus.  The position will manage the MDS process in compliance with CMS regulations, MCR, Medicaid guidelines and state reimbursement models.

    What you’ll do:
    • Oversee and coordinate the full MDS 3.0 assessment process

    • Oversee MDS Staff

    • Manage quality measures

    • Ensure timely and accurate MDS submissions per federal and state guidelines

    • Review, validate, and certify resident assessments for accuracy and compliance

    • Participate in and lead interdisciplinary care plan meetings

    • Evaluate resident physical, psychosocial, and cognitive status

    • Document assessments using the Electronic Medical Records system

    • Monitor and manage PDPM, case mix index (CMI), and ICD-10 coding

    • Lead Restorative Nursing Programs and assess their effectiveness

    • Maintain knowledge of Medicare, Medicaid, OBRA, and HIPAA compliance

    • Participate in survey readiness and reviews

    • May be required to fill in as an RN when required

    What you’ll bring:

    • Active Registered Nurse (RN) license in Kentucky (KBN-verified)

    • Minimum 2 years of clinical nursing experience

    • At least 1 year of long-term care or skilled nursing experience

    • Direct experience completing MDS assessments

    • Familiarity with RAI process, Medicare/Medicaid reimbursement, and state case mix models

    • Strong knowledge of CMS regulations, quality measures, and restorative care

    • Proficiency in EHR and MDS documentation platforms
