17 bed skilled nursing community


Role:  Works within the Community Life Team.  Implements the day-to-day operations of Social Services of the community to ensure the maximum levels of resident leisure contentment, as well as physical, mental and psychological well-being.  This is accomplished by taking into account Juniper Village’ philosophy, hallmarks, policies and procedures as well as current federal, state and local regulatory standards.

 

Responsibilities:

·       Ensure compliance to Juniper Village’ philosophy and hallmarks as well as all written policies and procedures that govern the operation of the Community Life Team.

·       Ensures resident safety at all times.

·       Embraces Culture Change and resident-centered philosophies

·       Ensure compliance with documentation and record keeping to meet current regulatory compliance standards.

·       Completion of a pre-admission assessment for potential admissions as assigned.  Preparation of pre-admission care plans as requested by DDM or Long-Term Care Options.

·       Complete initial psychosocial assessment and social history for assigned residents.

·       Complete quarterly and as needed psychosocial assessments for assigned residents.

·       Create a care plan with the input of the resident, responsible party and other care team members to address needs identified in the psychosocial assessment and any goals the resident desires to set.

·       Complete progress notes related to assessment, care planning, and significant happenings involving the resident.

·       Conduct interviews related to occurrence or incident investigations as assigned.

·       Complete smoking “At-Risk” Checklist after observation of resident smoking. 

·       Carry out QAPI roles and responsibilities as assigned.

·       Follow established policies and procedures in support of QAPI efforts.

·       Conduct therapy groups with residents who have cognitive, behavioral or mental health issues.  Conduct 1:1 therapeutic session with residents regarding their mental health issues, behavior, or adjustment issues.

·       Conduct psycho-educational groups related to current resident needs (i.e. anger management, life skills, relationship skills, social skills.

·       Provide crisis intervention to include use of de-escalation techniques, and determine when a resident requires a higher level of intervention (i.e. call 911, possible psychiatric hospitalization)

·       Assess resident’s mental status, communication skills, mood, behavior and psychosocial needs.

·       Develop care plan for resident’s needs related to cognitive, communication, mood, behavior and/or psychosocial needs.

·       Organize, prioritize, and meet paperwork deadlines.

·       Advocate for resident and arrange for medically related social services as listed in the CMS and State Regulations including but not limited to:

o   Ancillary Needs Assessment and Referral

o   Resident education about resident rights and responsibilities

o   Resident education about advance directive options

o   Discharge planning for residents who do not meet level of care criteria or desire to move to another placement.

o   Assistance with resident re-determination for Medicaid, application for Medicaid, or obtaining of other financial assistance needed.

·       Advocate for residents to have their life long preferences, interests, and goals supported by the staff and community as possible.

·       Facilitate family meetings and/or care conferences quarterly and on an as needed basis.

·       Conduct resident council and/or community meetings such that residents have the right to express ideas, concerns, complaints, etc.  Ensuring follow-up to these items identified.

·       Follow up on resident complaints or concerns until they are satisfied with the result.  Assisting them in filing a grievance if a solution isn’t found.

·       Assist in the planning and implementation of the In-Touch Orientation and in-service training, touring, and presentations.

·       Provide tour training and customer service training to staff.

·       Make written and weekly oral reports and meets as directed with the Social Services Director concerning social services.

·       Work in concert with other members of the Leadership Team.

·       Submit recommendations to the Social Services Director for budget completion, equipment, and supplies.

 

 

Educational/Vocational Requirements:

·       A High School Diploma, a Bachelor’s Degree in Social Work, Therapeutic Recreation, Psychology, Gerontology, or another related field is preferred.

·       Team member will possess one of the following certifications/ licensures within one year of hire:

o   Certified Social Worker (C.S.W.)

o   Licensed Clinical Social Worker (L.C.S.W.)

o   Licensed Practical Counselor (L.P.C.)

o   Certified Addictions Counselor (C.A.C. II or higher)

·       Previous experience in healthcare or service-related setting.

·       Knowledge of the physiology and psychology of the older adult.

·       Experience in program design, planning and implementation is desired.

·       Certification in CPR and First Aid

·       Willing to travel

·       Must have a valid State Drivers License

·       Obtaining and keeping current a CPI certification.

General Requirements:

·       Must be able to read, write, speak, and understand the English language.  Must be computer literate.

·       Must not pose a direct threat to the health or safety of other individuals within the community.

·       Attends and participates in mandated yearly training programs

·       Must be able to manage time efficiently so as to achieve goals and meet resident immediate needs.

·       Adheres to all Human Resources policies, procedures, and code of conduct.

·       Respects confidentiality of information and Resident’s rights.

·       Perform job in a safe manner, using proper body mechanics, infection control, etc. 

·       Responsible for proper care of all property, equipment, supplies and financial assets involved in the performance of the job.

·       Ability to conduct interviews with residents, 1:1 and group activities both planned and spontaneous.

·       Ability to conduct psycho-educational groups related to current resident needs (i.e. anger management, life skills, relationship skills, social skills).  Completion group skills class in-house or from approved outside agency is required with in 1 year of employment.

·       Ability to remain calm in a crisis, use de-escalation techniques, and determine when a resident requires a higher level of intervention (i.e. call 911)

·       Ability to write clear and professional plans for resident activities.

·       Ability to organize, prioritize, and meet paperwork deadlines.

·       Ability to motivate residents and be spontaneous in provision of activities.

·       Ability to work with various staff personalities in a professional teamwork approach to care.

·       Perform other duties as required

 

 

Physical Requirements and Working Conditions:

·       Works in an office as well as throughout the community and its premises

·       Moves intermittently during working hours.  Movement includes, but is not limited to standing, walking, sitting, pulling, lifting, reaching, carrying, pushing, climbing, kneeling, stooping, and twisting.

·       Must be able to push, pull, move and/or lift a minimum of 30 pounds to a minimum height of 5 feet and be able to push, pull, move, and carry such weight.

·       Is subject to frequent interruptions

·       Must possess sight/hearing senses or use prosthetics that will enable these senses to function so that the requirements of the position can be met.

·       Must be able to assist in the evacuation of residents during emergency situations.

·       Able to communicate intentions and directions to residents, staff, families, and others as needed.

·       May be subject to hostile and emotionally upset residents, family members, staff, and visitors.

·       Works beyond normal working hours and on weekends and holidays.

·       Is subject to call back during emergency conditions

·       May be subject to the handling of and exposure to hazardous chemicals.

·       Is subject to exposure to infectious waste, disease, conditions, etc. including tuberculosis, the AIDS and Hepatitis B viruses

·       Is subject to injury from falls, burns from equipment, odors, etc. throughout the workday as well as reactions from duct, disinfectants, and other air contaminants.
