RN Case Manager

Vitality Living is seeking a results-driven RN Case Manager to support Vitality Living of Park Rapids in a fast-paced, execution-focused environment.  This role is ideal for a nurse who can move quickly, lead confidently, and ensure high-quality, compliant care.

Schedule: 32 hours/week

No over night call - after-hours supported by ACS

No weekends and no holidays!

Key Responsibilities:

• Complete nursing assessments (initial, routine, change-in-condition)
• Develop and manage resident care plans
• Ensure accurate, compliant documentation (144G)
• Provide clinical leadership and support to care teams
• Collaborate to drive quality outcomes and resident safety

Qualifications:

• Active Minnesota RN license (required)
• 3-5 years skilled nursing or long-term care experience (required)
• Strong knowledge of assessments, care planning, and compliance
• Ability to execute quickly and work independently
