We are seeking a motivated Registered Nurse (RN) to join our team as a Care Manager. This leadership role is primarily field-based and focuses on overseeing patient care, supporting and training staff, and ensuring clinical excellence. You will play a key role in maintaining high standards of care while building strong connections with both clients and caregivers. A company vehicle is provided for travel to patient homes.

• Conduct patient admissions and ongoing supervisory visits

• Train, mentor, and support field staff

• Develop and oversee patient care plans and physician orders

• Provide direct patient care as needed

• Ensure compliance with healthcare regulations and company policies

• Build and maintain strong relationships with clients and caregivers

• Collaborate with the Director of Nursing (DON) on clinical and office-related tasks

• Current Registered Nurse (RN) license in Illinois

• Minimum 1 year of experience in home health and pediatrics

• Strong clinical judgment and leadership skills

• Excellent communication and interpersonal abilities

• Knowledge of healthcare regulations and compliance standards

• Ability to work independently and as part of a team

• Valid driver’s license and clean driving record
