Concern Housing is seeking a Program Supervisor for the CTAP Program to be responsible for the overall supervision of the CTAP program and responsible for managing the program in a manner that promotes client growth, safety, and the effective delivery of rehabilitation services.

Position: Program Supervisor - Apartment Treatment Program
Location: Port Jefferson, NY
Schedule: Tuesday - Friday; 9am - 5pm & Saturday; 8am - 4pm
Salary: $68,000/yr.

Duties and Responsibilities:

- Supervise and coordinate the provision of direct care services; manage caseloads accordingly; provide direct support to clients during
  times of staffing challenges.
- Supervise program staff on a weekly basis; provide staff training as needed.
- Ensure program compliance with agency standards of care as well as regulatory guidelines, including OMH’s licensed program
  requirements and Medicaid billable services, such as:
  the provision of 6 services monthly to each client
- Service Plans and Reviews according to due date requirements
- The obtainment of Physician Authorizations according to due date requirements
- Maintain up-to-date tracking systems of all documentation due dates to ensure compliance.
- Complete periodic staff evaluations; participate in hiring and dismissal of staff.
- Ensure staff remain in compliance with agency-issued documentation sign-offs and Relias trainings.
- Provide 24/7 crisis intervention support.
- Remain current with all Agency required compliance documents and trainings.
- Other tasks as assigned by the Director of Community Residence Programs.

Requirements: Master’s Degree in relevant field preferred. The individual in this position must display effective leadership and supervisory skills, and have the ability to make sound judgments regarding client care.
