Position Overview:
We are seeking a skilled and compassionate Registered Nurse (RN) to join our hospice care team. The ideal candidate is a strong communicator who thrives in an autonomous role and is passionate about providing holistic, patient-centered care.
Key Responsibilities:
Conduct initial and ongoing patient assessments to determine physical, emotional, and spiritual needs.
Develop and implement individualized care plans in collaboration with physicians and the interdisciplinary team.
Provide direct skilled nursing care, pain and symptom management, and emotional support to patients and families.
Educate family members on end-of-life care, medications, and comfort measures.
Document all visits accurately and timely in accordance with agency policies and regulatory standards.
Participate in on-call rotation and interdisciplinary meetings.
Qualifications
Active Registered Nurse (RN) license in Pennsylvania - required
Minimum 2 year of professional hospice experience - strongly preferred
Demonstrated ability to perform thorough clinical assessments, prioritize effectively, and manage complex caseloads with autonomy
Exceptional communication, critical thinking, and documentation skills
Valid driver’s license, reliable transportation, and willingness to travel to patient homes as needed
