Licensed Nurse MDS/MMQ Coordinator - ON SITE PDPM TRAINING PROVIDED

***On-site PDPM training and support will be provided for this position***

Den-Mar Health and Rehabilitation Center, located in Rockport, MA, is seeking an experienced Licensed Nurse (RN/LPN) MDS/MMQ Coordinator to join its team.

Responsibilities includes but is not limited to: overseeing and coordinating the developments and completions of the resident assessment (MDS/PPS) and MMQ assessments in accordance with current Federal and state regulations, coordinating and leading the development of a comprehensive assessment and care plan for each resident as chairperson of the facility's Interdisciplinary Team, and ensuring the face validity of all Minimum Data Sets before electronic submission and combine the timely electronic submission of all MDS.

Licensed Nurse (RN/LPN) MDS/MMQ Coordinator Required Experience & Education:

• Licensed Nurse (RN/LPN) in the state of Massachusetts
• Previous experience in a long-term care facility as a facility MDS/MMQ Coordinator/Case Manager
• Must be knowledgeable of general, rehabilitative and restorative nursing and medical practices and procedures and laws, regulations and guidelines governing long-term care.
• Computer skills-Windows applications, computer experience. PointClickCare (PCC) experience preferred.
