Social Services Director

Directs and supervises the resident social service program. Assists in the coordination of resident transfers. Handles inquiries about admissions and interviews each resident and family on admission, including admission paperwork. Coordinates services with other departments. Completes admissions paperwork as assigned. Formulates a written plan of care for each admitted resident. Participates in the development and review of the residents Care Plan. Refers residents and families to appropriate community resources as necessary.
