Community Roots Navigator

Kelowna, BC Temporary $24.36-$28.01/hour

CMHA Kelowna is hiring a full-time term Community Roots Navigator. This is a 12-month term position that offers 37.5 hours a week with a typical work week of Monday – Friday, 8:00am – 4:00pm.

 

WHO IS A COMMUNITY ROOTS NAVIGATOR?

The Navigator provides proactive outreach and navigation support to individuals seeking access to a wide range of community-based services. This position is instrumental in guiding participants toward appropriate resources, helping them build practical life skills, and supporting their journey toward greater independence and community inclusion. Working through Synergy, a resource hub operated by Community Roots, the Navigator connects individuals with essential programs and services. This role also contributes to the coordination and delivery of services under a contract with Community Living BC (CLBC), ensuring the individual receives personalized, inclusive, and strengths-based support.
 

WHAT DOES A COMMUNITY ROOTS NAVIGATOR DO?

  • Conduct interviews with participants to identify needs and challenges
  • Collaborate with health and social service providers to develop tailored case plans
  • Document participant needs and assists in selecting appropriate programs
  • Receive and addressing participant feedback, inquiries, and complaints
  • Facilitate the Synergy program on a biweekly basis
  • Advocate for participants in areas such as housing disputes, financial aid, and volunteer opportunities 
  • Identify and assess potential emergency situations involving the CLBC participants and respond appropriately with short-term strategies as needed. 
  • Assist the CLBC participant in developing physical, social, emotional, and life skills goal setting, including personal hygiene, meal planning, and financial management 
  • Provide the CLBC participant guidance on maintaining appointments and improving interpersonal skills 
  • Identify and inform participants about available social, economic, recreational, and educational resources in the community 
  • Facilitate access to necessary services to meet participant needs 
  • Maintain accurate records, including case files, progress reports, data management, and daily logs
  • Performs other related duties as assigned 

YOUR STRENGTHS & EXPERIENCE:

  • Diploma or degree in Community/Social/Human Services required 
  • Valid Class 5 driver’s license and reliable vehicle
  •  Criminal Record Check clearance in the Vulnerable Sector 
  • Certificates in First Aid, CPR and Naloxone considered an asset 
  • Minimum 1 year of recent, related experience or a combination of education, training, and relevant experience 
  • Ability to establish and maintain rapport with participants
  • Communicates effectively, both verbally and in writing
  • Ability to work independently and collaboratively with a team
  • Maintain confidentiality and ethical standards in all interactions
  • Demonstrate cultural competence and sensitivity
  • Ability to maintain consistent professional boundaries 
  • Proficient in Microsoft Outlook, Word and Excel
  • Understanding of Harm Reduction theory and demonstrated ability to implement harm reduction strategies
  • Understanding of Shelter Diversion principles
  • Understanding of working with a Trauma Informed lens
  • Ability to analyze and assess participant situations and needs critically
  • Strong organizational and prioritization skills
  • Capacity to observe and recognize changes in participant behavior and circumstances
  • Demonstrate conflict resolution and problem-solving abilities
  • Ability to be regularly exposed to individuals who are struggling with and/or recovering from trauma, drug/alcohol use, homelessness, mental health challenges and/or mental illness
  • Ability to travel between facilities and other locations on an occasional basis
