Nurse Practitioner - Wound Care (PRN)

The Nurse Practitioner – Wound Care plays a critical role in assessing patients. This position requires a wound care certified nurse practitioner who is experienced, team-oriented, and able to provide expert wound care in the office or nursing home setting according to the practice guidelines of the state of Tennessee or Mississippi on an as-needed basis. The nurse practitioner will provide and oversee wound care, diagnose, and treat wound care patients following evidence-based guidelines, monitor clinical quality of care, and ensure regulatory and company compliance. Qualified candidates will have strong knowledge of evidenced-based wound care practices, the use of skin substitutes and other strategies to improve targeted outcomes. The wound certified nurse practitioner will work closely with the assigned team to monitor outcomes, clinical efficiency, appropriateness/quality of care and supervise care.

Duties and Responsibilities:

• Demonstrates proficiency in performing the full scope of wound care and management, including patient health interviews, review of medical/surgical health history, physical assessment, ordering and interpretation of diagnostic testing, diagnosing health problems, managing health care, providing health teaching and counseling, initiation of referrals, following all appropriate guidelines for indicated treatment and follow-up scheduling, collaborating with assigned team members and other health care providers, and maintaining electronic health records.

• Complete documentation that meets regulatory guidelines and ensures compliance with documentation in the application-based electronic patient record for clinical care instructions, communications, wound care supply orders, and diagnostic testing results.

• Prescribes appropriate treatment orders in accordance with the guidelines for wound dressings, laboratory testing, cultures, imaging, specialty/offloading, and pressure reducing devices.

• The advanced practice nurse may prescribe non-scheduled legend drugs, initiate, monitor, alter or order drug therapies based on educational preparation, clinical experience, and state guidelines/scope of practice either in collaboration with a physician provider or under the supervision of a physician provider.

• Implement and maintain professional patient communications during in-home visits and via the application-based electronic patient record.

• Maintains and ensures regional compliance with all regulatory and company standards.

• Completes and monitors appropriate notices for discharge including referral to the case manager for post-healing follow-up within the assigned region when indicated.

Minimum Qualifications:

  • Current Mississippi State Nursing License as a APRN or Physician Assistant.
  • Certification as a Family Nurse Practitioner through the AANP or ANCC or Certification as a Physician Assistant through the NCCPA.
  • DEA license, or willingness to obtain upon hire.
  • Certification in wound care (CWCA, CWCN, or equivalent)
  • Minimum of two (2) years of experience in wound care management, with proficiency in wound assessment.
  • Strong interpersonal and communication skills, with the ability to effectively collaborate with patients, families, and team members.

Physical Requirements:

The physical activities of this position may include climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motion. Must have good balance and coordination. The physical requirements of this position are medium work - exerting up to 50 lbs. of force occasionally and/or up to 25 lbs. of force frequently. The Nurse Practitioner – Wound Care is required to have close visual acuity including color, depth perception, and field of vision to perform an activity, such as assessing patients, and analyzing data and figures; transcribing; viewing a computer terminal; or extensive reading. Frequent invasive and non-invasive patient contact. Exposure to patient body fluids as well as exposure to hazardous or poisonous materials.
