Key Responsibilities:
· Complete nursing assessments (initial, routine, change-in-condition)
· Develop and manage resident care plans
· Ensure accurate, compliant documentation (144G)
· Provide clinical leadership and support to care teams
· Collaborate to drive quality outcomes and resident safety
Qualifications:
· Active Minnesota RN license (required)
· 3-5 years skilled nursing or long-term care experience (required)
· Strong knowledge of assessments, care planning, and compliance
· Ability to execute quickly and work independently
