LEAD OUTREACH SPECIALIST

PRINCIPAL RESPONSIBILITIES
The LEAD OUTREACH SPECIALIST will work within a Street-based Multidisciplinary Team (MDT) to engage, assess, and develop permanent housing plans with individuals experiencing homelessness, utilizing best practices to provide intensive case management services and crisis intervention. They will collaborate with various community-based organizations to ensure appropriate service connections are obtained and provide follow-up referral/service tracking to individuals experiencing homelessness living on the streets.

- Serves and protects Christ-Centered Ministries (CCM) practices by adhering to professional standards, policies and procedures, federal, state, and local requirements;
- Maintains accurate documentation of service objectives and outcomes as well as other services in accordance with Federal, State, County, and Christ-Centered Ministries;
- Secures client information and maintains client confidence by completing and safeguarding data; keeping client information confidential and in compliance with HIPAA standards;
- Maintains a safe, secure, and healthy work environment by establishing and following standards and procedures, complying with legal regulations including acting as a mandated reporter.
LEAD OUTREACH SPECIALISTS ARE EXPECTED OF THE FOLLOWING:
- Provides mobile, field-based outreach to people experiencing homelessness living on the streets and in homeless encampments.
- Works collaboratively with other outreach entities in the community (LAHSA’s ERT, LAPD, DMH, VA).
- Onboards newly hired staff and registers them for required and additional trainings.
- Supports and provides technical assistance with CES to reduce street homelessness by linking program participants to mental health, health services, housing services, and other supportive services for which they are eligible.
- Drives and intervenes with a mobile multi-disciplinary team (includes medical professional, (3) outreach specialists, and clinical outreach specialists) to ensure homeless adults on the streets are triaged to needed medical care and health care assessment.
- Ensures direct crisis intervention with consultation with Clinical Supervisor, LCSW to support the multi-disciplinary team by utilizing a Harm Reduction and Housing First approach.
- Provides daily summarized reporting to the Program Manager and maintains thorough and accurate records in both written form and through assigned electronic database.
- Additional tasks, projects, and responsibilities as assigned by Program Management/Operations Leadership staff.
- Other duties as required.
QUALIFICATIONS & PERSONAL ATTRIBUTES DESIRED
EDUCATION & EXPERIENCE:
- High School diploma or GED. Some college preferred;
- Two (2) years of experience working with people experiencing homelessness and managing service providers or have a minimum of two (2) years of experience providing direct health, behavioral health, substance use, or counseling or ICMS-SBE,
- Knowledgeable, experienced, and committed to housing and homeless issues is essential;
- Demonstrated creative problem-solving skills;
- Knowledge of crisis intervention, Part-R, CPR, and First Aid;
- Must be proficient in the following computer applications: Microsoft Word and Excel;
- Required valid CA driver license, have reliable transportation, and a good driving record;
- Bilingual (Spanish/English) highly desirable.
PHYSICAL REQUIREMENTS/WORK ENVIRONMENT:
There is some walking, bending, and kneeling. Must be able to participate in moderate lifting of 30 or more pounds (suitcases, packages, etc.). This position must be able to support client weight in case of emergency situation or client disability requiring assistance in walking, etc. There is a risk of exposure to bodily fluids, infections, contagious / blood-borne diseases, and chemicals (cleaning agents), an exposed risk to varying, unpredictable situations (i.e., client behavior).

- Excellent oral and written communication skills and ability to present persuasively.
