Speech-Language Pathologist (SLP) – Skilled Nursing Facility

Location: Orange, NJ

A skilled nursing facility in Orange, NJ is seeking a qualified Speech-Language Pathologist (SLP) to provide evaluation and treatment services for short-term rehabilitation and long-term care residents. This role is ideal for an SLP with experience in skilled nursing or post-acute care who is comfortable managing medically complex patients.

Key Responsibilities

• Evaluate residents for speech, language, cognitive, and swallowing disorders
• Develop and implement individualized treatment plans based on clinical findings
• Provide dysphagia management, cognitive-communication therapy, and speech/language interventions
• Collaborate with nursing, therapy, dietary, and medical staff to support resident care plans
• Monitor resident progress and adjust treatment plans as appropriate
• Document evaluations, progress notes, and discharge summaries in accordance with Medicare and facility standards
• Participate in care plan meetings and interdisciplinary team discussions
• Support safe discharge planning and continuity of care

Required Qualifications

• Licensed Speech-Language Pathologist (SLP) in the state of New Jersey
• 1–2+ years of experience in a skilled nursing facility, long-term care, or post-acute rehab setting preferred
• Strong knowledge of dysphagia management and skilled documentation requirements
• Familiarity with Medicare Part A / Part B guidelines
• Ability to manage a productive caseload in a skilled nursing environment
• Strong communication and collaboration skills
