Comprehensive Case Manager (Bilingual in English and Spanish required)

 

Salary starting at $53,000/year.  Salary is determined based on experience.  The Comprehensive Case Manager provides trauma-informed, ongoing case management for children and families impacted by abuse. This role includes conducting trauma and suicide assessments, developing Family Service Plans, monitoring referrals, and ensuring families receive consistent, individualized support throughout the CAC process.  

 Essential Functions

  • Administer and document trauma and suicide assessments to identify trauma related symptoms and support clinical referrals.  
  • Collaborate with clinical staff to determine appropriate next steps based on assessment results. 
  • Develop and maintain Family Service Plans (FSP) that reflect evolving family needs. 
  • Classify families using tiered support levels and adjust as needs change. 
  • Provide psychoeducation to caregivers on trauma responses, investigation processes, and abuse dynamics. 
  • Monitor referrals for therapy, medical, and community resources; follow up on barriers and attendance. 
  • Conduct structured check-ins during therapy waitlist periods and stabilization phases. 
  • Facilitate family support meetings for escalated cases awaiting therapy as needed. 
  • Provide court orientation, court accompaniment, and case updates as needed. 
  • Collaborate with Multidisciplinary team partners to ensure families are supported throughout all stages. 
  • Facilitate psychoeducation groups for children and caregivers. 
  • Participate in the coordination of direct service programs such as Back-to-School drives and holiday support (e.g., Christmas program) 
  • Participate in required training, continuing education, and family advocacy peer review. 
  • Participate in the On-Call rotation, including after-hours, weekends, and holiday availability. 
  • Maintain ethical standards, ensuring confidentiality, professionalism, and advocacy aligned with the Centers code of ethics and policies. 
  • Maintain proper documentation in CACNT electronic health record/client files 
  • Other duties as assigned.

Travel and Expected Hours of Work

  • This position is based in the Lewisville, TX office.  The candidate must be willing to work at all offices (Bridgeport, Lewisville, and Denton) when agency needs arise.  Work schedule includes two evenings per week, with shifts extending up to 8:00 PM to accommodate family availability. 

Education and Training

  • Bachelor’s degree in Social Work or a related field required.

Experience

  • A minimum of 2-years of experience in victim advocacy, case management, or trauma-related services required.   
  • Bilingual in English and Spanish required.
