What You’ll Do
As a Consumer Care Specialist, your role ensures care stays organized, consistent, and person-centered.
Coordinate Essential Care: Schedule, track, and manage medical, dental, vision, and wellness appointment to ensure continuity of care.
Keep information Organized & Compliant: Maintain accurate records, monitor documentation, and ensure information stays current, secure, and aligned with requirements.
Contribute to Direct Services: Provide direct care services three says per week to stay connected to the consumer experience and understand the impact of services firsthand.
Communicate & Advocate: Work closely with individuals, families, providers, and internal teams to ensure appointments and care plans run smoothly.
Support Community Inclusion: Help connect people to resources, events, and opportunities that promote wellbeing and participation.
What We're Looking For
Education & Experience:
Relevant degree (preferred). High school diploma or equivalent required.
Experience in healthcare coordination, scheduling, records management, or social services is a plus.
Skills:
Strong organization and attention to detail. Clear, professional communication.
Able to work independently and manage multiple priorities.
Passion for Inclusion: A genuine desire to support individuals with disabilities.
Requirements: Valid driver’s license and reliable transportation.
