Skilled Nursing Facility Hospital Liaison

Location: Winter Park, FL

We’re working with a well-established skilled nursing facility in the Orlando area that is looking to add an experienced Hospital Liaison to their team. This role is critical to driving referrals, maintaining strong hospital relationships, and ensuring smooth transitions of care from acute settings into post-acute care.

What You’ll Do:

• Serve as the primary point of contact between hospitals, case managers, discharge planners, and the SNF
• Build and maintain strong referral relationships with local hospitals and healthcare partners
• Coordinate patient referrals, clinical reviews, and admissions with the facility team
• Communicate clearly and professionally with families regarding post-acute placement
• Track referral activity and conversion metrics
• Represent the facility in a positive, professional manner in hospital settings

What They’re Looking For:

• Previous experience as a Hospital Liaison, Clinical Liaison, or Admissions Liaison in a SNF or post-acute setting
• Strong relationships with Orlando-area hospitals preferred
• Clinical background (RN, LPN, or social work) is a plus, but not required
• Excellent communication, follow-up, and organizational skills
• Ability to work independently and manage multiple referrals at once
• Knowledge of Medicare, managed care, and discharge planning processes
