Team: ACTT (Assertive Community Treatment Team)
Location: Community-based / Office / Field
Schedule: Monday–Friday with on-call rotation, evenings/weekends as required
Travel: Frequent local travel

Role Overview:
The ACTT Qualified Professional (QP) delivers intensive, community-based mental health services to adults with severe, persistent mental illness. You’ll provide recovery-focused, person-centered care, helping members with stabilization, independent living, and community integration while following NC DHHS, Medicaid, and CARF standards.

Key Responsibilities:

  • Conduct assessments, treatment, and crisis intervention in home, community, and office settings.
  • Support daily living skills, medication adherence, symptom management, employment, housing, and social integration.
  • Develop, implement, and monitor Person-Centered Plans (PCPs) with measurable goals.
  • Participate in daily ACTT team meetings, sharing caseload responsibilities and collaborating with psychiatrists, nurses, substance use specialists, and peer support staff.
  • Complete timely, audit-ready documentation in the EHR; maintain compliance with Medicaid and service definitions.
  • Provide on-call crisis support, respond to psychiatric emergencies, and facilitate transitions of care.
  • Coordinate services with external providers and community resources.

Requirements:

  • Bachelor’s degree in Psychology, Social Work, Counseling, or related field.
  • Meet NC DHHS Qualified Professional criteria.
  • Minimum 2 years working with individuals with mental illness; ACTT or community-based experience preferred.
  • Preferred: LCSW, LCMHC, LCAS, or equivalent licensure.
  • Valid driver’s license, reliable transportation, and current auto insurance.
  • Strong crisis management, documentation, and team collaboration skills.

Environment:

  • Combination of office, community, and home-based service delivery.
  • Exposure to unpredictable environments; requires flexibility, travel, and ability to respond to crises.
